Special Agent Arnold Samuels has discovered the smallpox attack has something to do with a man he killed in cold blood, but he needs more information. He visits a Mafia soldier he had tipped off years earlier during the FBI's infiltration of organized crime families. The investigation takes on a new focus when Samuels learns of Mike Greco, the son of the man he had killed. A raid of Greco's Yonkers home leads a blaze of bullets and the taking of two hostages.
