A NOTE ON METHODThe Threshold Bureau's formal documentation practice requires that every significant finding be recorded in two parts: what was achieved, and what the achievement cost. What was discovered. What the discovery asked of the person who made it. This practice did not originate as a safety protocol. It originated as honesty. The researchers documented here did not die because they were careless.
They died because they were thorough. The names of the deceased are real, used with permission of their families, who were told a version of the truth and who, in every case, deserved the complete version. The complete version is this. A note on the timestamp that recurs throughout this record: 14:23:00 is the exact moment - to the second - at which Dr. Yara Osman's dive camera recorded the stillness that preceded her death.
It is also the time at which the Bureau's founding memo was signed, a coincidence that the Bureau's lead analyst spent three weeks trying to verify as meaningful before concluding it was not. He was wrong about that. This is also part of the record.
