Life has surely changed since Tanner and Peyton Elliott had a chance meeting with Alena Fox in that bar, all those months ago. In that time, she has become their free-use live-in housemate, and then evolved into a second wife for the couple, in romance if not in the eyes of the law. Now, they are experiencing new fantasies, new parts of themselves, parts they never predicted or knew were there before Alena came into their lives.
They have travelled to the Mediterranean island of Vulcano to meet with Adriana LaCroix, a transgender adult film star who has the ability (and physical equipment) to satisfy a burgeoning fantasy for Tanner Elliott, if he has the courage to act on his fantasies.
