Drawn from the domains of aviation, engineering, medicine, military operations, and energy, the cases cover centuries of high-risk decision-making.
The thinking failures they reveal are not random. They are patterned, predictable, and preventable. The book is organized around four categories of intellectual failure: the foundation of clear thought, the quality of evidence, the direction of attention, and the character of the thinker. The fifth chapter examines the accident at Three Mile Island -a catastrophic event where the disaster stopped-as a combination of all 15 standards under extreme pressure.
A Postlude presents an unedited exchange with an AI language model on whether these standards can survive in an information ecosystem increasingly shaped by systems that generate confident answers without knowing whether those answers are true. The reader is invited to evaluate it by the same standards evaluated throughout. For engineers, pilots, physicians, military officers, and anyone who makes consequential decisions under pressure: the framework is demanding, the evidence is real, and when lives depend on your thinking, good intentions aren't enough.
Peter H. Tyson is a retired U. S. Navy Commander with 21 years of service as a helicopter pilot, developmental test pilot, and Aviation Engineering Duty Officer. A Naval Test Pilot School graduate with BS and MS degrees in aeronautical engineering, he spent 15 years after leaving the Navy teaching critical thinking to the naval aviation workforce at NAVAIR/Patuxent River, reaching more than 10, 000 students.
He is the founder of Paradise View Press.
